Permission to Skate Forms issued by SCMH is only valid for participation with the Hamilton Jr. Bulldogs. Proof of Stoney Creek residency in the form of a driver's license, passport, or other government ID, is required when picking up the PTS form. The PTS forms may be picked up when pre-registering for the Jr. Bulldogs tryouts.
